By GulfInside | United States | Updated October 20, 2026. Medicare’s annual open enrollment period is underway, running from October 15 through December 7, 2026. People already enrolled in Medicare can review and change Medicare Advantage and prescription drug coverage for 2027. Most changes submitted during this window take effect January 1, 2027. The rules are described on Medicare.gov’s official plan enrollment guide.
What can you change during Medicare open enrollment?
- Switch from Original Medicare to a Medicare Advantage plan if eligible.
- Switch from Medicare Advantage back to Original Medicare.
- Switch between Medicare Advantage plans.
- Join, drop or switch a standalone Medicare Part D prescription drug plan, subject to your coverage arrangement.
Medicare’s annual October–December open enrollment is different from the initial enrollment period for people first eligible for Medicare and from the January 1–March 31 Medicare Advantage open enrollment period. Not everyone can make every type of change in every period.
Medicare Advantage vs Original Medicare: what to compare
Coverage structure
- Original Medicare
- Part A and Part B administered by Medicare; optional separate Part D and Medigap
- Medicare Advantage
- Private Medicare-approved plan providing Part A and Part B benefits, often with Part D
Doctors and hospitals
- Original Medicare
- Generally any provider accepting Medicare nationwide
- Medicare Advantage
- May have network and referral requirements
Prescription drugs
- Original Medicare
- Typically a separate Part D plan
- Medicare Advantage
- Often included, but verify exact plan
Annual out-of-pocket limit
- Original Medicare
- No general annual limit for Part A/B spending without supplemental coverage
- Medicare Advantage
- Plan-specific annual limit on covered Part A/B services
Supplemental benefits
- Original Medicare
- Not automatically included
- Medicare Advantage
- May include dental, vision or hearing benefits with restrictions
These are general program differences. Costs, provider access and specific benefits depend on the plan and location. See Medicare’s comparison of coverage options.
Seven-step checklist before December 7
- Read your Annual Notice of Change. Check what your current plan says will change for 2027, including premiums, deductibles and covered services.
- List prescriptions. Record medication name, strength, frequency and preferred pharmacy, then check the 2027 formulary and any prior authorization or step-therapy rules.
- Confirm providers. Check each doctor’s and hospital’s network status with the plan and the provider, particularly for ongoing care.
- Compare total annual costs. Look beyond monthly premiums to deductibles, copays, coinsurance, drug tiers and the out-of-pocket maximum where applicable.
- Check travel and geography. Confirm routine and emergency coverage if you spend time outside your plan’s service area.
- Review extra benefits carefully. Dental, vision, hearing and transportation allowances may have eligibility, network and usage limits.
- Save confirmation. Keep the enrollment confirmation number and check the effective date before January 1.
How to compare Medicare plans for 2027
Use the official Medicare Plan Compare tool. Enter your ZIP code and review the plans available in your area. Add your prescription drugs and preferred pharmacies where appropriate, and inspect plan details before applying. Plan availability and pricing vary by county and sometimes by ZIP code, so national averages cannot substitute for your own plan comparison.
What if you do nothing?
In many cases, an existing plan continues into the next year if it remains available and you remain eligible, but its costs, coverage, drug formulary and provider network may change. Some plans exit markets or change their terms. Read the Annual Notice of Change rather than assuming nothing will change.
What about 2027 Medicare premiums?
Do not assume a specific 2027 Part B premium or deductible unless it has been formally announced by CMS. The official Medicare site publishes current premiums and deductibles at Medicare costs. Beneficiaries whose Part B premiums are deducted from Social Security should also review their personalized SSA benefit notice once available.

Be cautious of unsolicited callers demanding a Medicare number or promising a cash benefit to enroll. Do not share Medicare or bank details with unverified callers.
